IMMUNIZATIONS

The State of Alabama Public Health Department has mandated that student immunization records must be up to date.  Immunizations not only secure the health of your child, but protect other children who may be at high risk for acquiring communicable diseases.

 

You will be sent a notice informing you of the expiration date on the your student's blue form. When you receive this notice call your health care provider as soon as possible to get the immunizations completed. Do not wait until the expiration date. We must have a new, updated BLUE form. Copies are not acceptible. 

 

Your cooperation in sending this completed form by the expired date to the school is greatly appreciated.  It may be turned into the nursing or counseling offices.
