Hello  Parents!!  My name is LeeAnne Logan, and I am the School Nurse for MBE.   I look forward to taking care of  the special health needs of your children.  I am on campus during school hours so if your child has any health concerns feel free to come by, email, or call me at 871-8191 ext. 7409.  My  office is on the main hall right across from Ms. Brewster's 2nd grade classroom.  I'm looking forward to a happy and healthy year at our school!!

If your child needs medication at school, you may download the provider parent authorization form below.  You will need to fill it out and sign it for "over the counter" medications.  For prescription meds, your physician will need to sign the form before medication can be administered.  All medication must be current,  labled, and in its original container.

 The end of the school year is here!!  I wanted to remind parents of students with medications in the nurse's office to come by and pick them up by May 24.  This includes over the counter and prescription meds.  Medications cannot be left over the summer and will be discarded. Students may not carry these medications home. Thank you for cooperation in this matter.
