Hello  Parents!!  My name is LeeAnne Logan, and I am the School Nurse for MBE.   I look forward to taking care of  the special health needs of your children.  I am on campus during school hours so if your child has any health concerns feel free to come by, email, or call me at 871-8191 ext. 7409.  My  office is on the main hall right across from Ms. Brewster's 2nd grade classroom.  I'm looking forward to a happy and healthy year at our school!!

If your child needs medication at school, you may download the provider parent authorization form below.  You will need to fill it out and sign it for "over the counter" medications.  For prescription meds, your physician will need to sign the form before medication can be administered.  All medication must be current,  labled, and in its original container.

Online Registration is now in progress.  Health Assessment forms and Contact Sheets will not be able to completed online.  Please print these out and fill them out and return them to your child's homerooom teacher before April 30, 2012.

Scoliosis Screening for 5th and 6th graders will be April 10.  A nurse from Children's Hospital will evaluate the student while standing and bending forward. It is suggested that girls where a camisole, tank top or bra underneath a loose fitting shirt.   Boys should wear a loose fitting tee shirt.

For Current 5th grade Students:  The new immunization law requires that all students entering 6th grade have a Tdap(Tetanus, Pertussis-"whooping cough") booster.  It may not be given until your child's 11th birthday.   Please contact your health provider regarding this booster. !!
