4. Request for Approval to Use Online Learning Resources that Require PII
Any time Personally Identifiable Student Information (PII) will be entered into a web based program the procedure below must be followed:
- Fill out the form at this link.
- The more information you can enter such as link to privacy policy, fields required, etc. will expedite the process.
- A committee will evaluate the site and review the site's data privacy policy.
- The person that completed the form and the technology coordinator involved, will receive a copy of the form marked approved or denied. Approval does not imply funding.
