4. Request for Approval to Use Online Learning Resources that Require PII

Any time Personally Identifiable Student Information (PII) will be entered into a web based program the procedure below must be followed:
  1. Fill out the form at this link.
  2. The more information you can enter such as link to privacy policy, fields required, etc. will expedite the process.
  3. A committee will evaluate the site and review the site's data privacy policy.
  4. The person that completed the form and the technology coordinator involved, will receive a copy of the form marked approved or denied.  Approval does not imply funding.
